Killingly Eye Care  – Office Policies

KEC pre-schedules all return appointments. These appointments are based on patient’s eye care needs, not insurance requirements. All patients are responsible to know and understand their insurance coverage.

KEC requires 24-hour notice of cancellation for all prescheduled appointments to avoid a $25 lost appointment fee.

Patients are responsible for all balances not paid by insurance. This include co-pays, deductibles or any procedures not covered by insurance. KEC participates with most insurance companies and accepts assignment.

All outstanding account balances must be paid in full before any contacts or glasses are ordered.

A deposit is required before glasses can be ordered. Only established patients with approved credit history may set up payment plans.

All contact lenses and eyeglasses must be paid in full at the time of pick up, unless a payment plan has been established.

A $25 fee will be charged for any checks returned for insufficient funds.
